The Challenge
After a routine cystectomy, the perioperative team discovered one of the surgical graspers used during the procedure was missing part of the black sheath that should enclose it. The team was unable to determine whether the sheath had been missing upon delivery or lost during surgery and potentially left in the patient’s body.
